The Desimone Levee on the Green River failed and is under repair. The hospital is above the flood zone and currently in no danger of flooding. We evacuated our Time Square and Kent Station Clinics and our Lind Avenue locations. Several locations are experiencing staffing issues due to severe traffic impacts. Impacted patients are being contacted to reschedule appointments. Please be safe, do not drive or walk through standing water, and call 9-1-1 if you need emergency evacuation assistance.

New patient access request

For access to a new patient where you were not designated as their provider for the patient to be included in your patient list in Provided Link, you may send us an inbasket message through Provider Link. We process access requests within 1 business day.
If you choose a paper method, send a fax on official letterhead, including the office name, address, and phone and fax numbers to 425.656.5008 with the following details :

  • Patient information:
    • Name
    • Date of Birth
    • Gender
  • VMC MRN # (if available)
  • Provider Link Stat Access statement in subject line:  “This is a STAT Access request for Provider Link”
  • Reason for access/request
