Share your story of a nurse who delivered extraordinary, skillful, compassionate care to you or a patient by filling out a nomination form today—it’s an important way of saying thank you to an extra special nurse who has made a difference in your life or another’s.

DAISY Award nominees personify Valley Medical Center’s remarkable patient experience. In addition to naming a DAISY Award honoree, each nominated nurse receives special acknowledgment from Valley Medical Center and The DAISY Foundation as our way of saying “thank you” to the unsung heroes in the nursing profession.

How to Nominate an Extraordinary Nurse

Patients, visitors, physicians, volunteers, nurses and other employees are encouraged to nominate a deserving nurse. There are two options for submitting a nomination:

  • Print and fill out a nomination form to mail to the address below, drop in a drop box located throughout the hospital, or email at the above email address.

Nomination Forms

  • Drop your nomination form off in a DAISY/BEE Award Nominee drop box located throughout the hospital

How are DAISY Award Recipients Honored?

Each DAISY Award honoree will be recognized at a public ceremony in his/her unit and will receive: a hand-carved stone sculpture entitled A Healer’s Touch, a DAISY Award pin and commemorative certificate.

About The DAISY Foundation

The DAISY Foundation was established in 1999 by the family of J. Patrick Barnes, who died of complications of the auto-immune disease, Idiopathic Thrombocytopenia Purpura (ITP), at the age of 33. During Pat’s eight- week hospitalization, his family was awestruck by the care and compassion his nurses provided—not only to Pat, but to everyone in his family. To honor Patrick’s memory and the extraordinary nursing care he received, his family created The DAISY Foundation.
