Filing a Tort Claim Against Valley Medical Center

Steps to File a Tort Claim

    • You can download the Standard Tort Claim Form for the State of Washington here.

  2. Gather Information
    • Collect all relevant documentation, including medical records, incident reports, and any correspondence related to the incident.
    • Note down important details such as the date, time, location, and names of those involved.

  3. Describe the Incident
    • Clearly explain what happened, including how and when the injury occurred. Provide as much detail as possible about the incident and the resulting damages.

  4. Include Supporting Documentation
    • Attach any supporting documents, such as medical bills, photographs, and witness statements, that can help substantiate your claim.

  5. Submit Your Claim

    • Submit your completed tort claim form and supporting documentation to Valley via email or mail:

        • Include your name and contact information and attach the Tort Claim form and supporting documents in PDF format.

  6. Timeline for Filing
    • It’s important to note that in Washington State, tort claims must be filed within three years of the incident. Failure to file within this timeframe may result in the forfeiture of your right to seek compensation.

  7. Await a Response
    • After submitting your claim, Valley Medical Center will review it and conduct an investigation. You will be notified in writing of the decision.
