UW Medicine Directories for Referrals

Two directories for UW Medicine clinics were recently updated:

Primary Care Clinics Directory: This is a resource if you receive referrals from PCPs at Kent/Des Moines, Federal Way or other UW Primary Care providers, or you otherwise would like to email or call those providers or clinic staff about shared patients.

UW Medicine Clinics Referral Directory: This contains the backline phone numbers and fax numbers for the specialty clinics at UWMC Montlake and Northwest, Harborview, Eastside Specialty Center, and other clinics.
Note: The Dept. # and Epic RFL details apply to UW Medicine, not to Valley. Some of the phone/fax contact info in the document for Valley is outdated. We are working with UW to update this information.
