Anticoagulation Clinic Name Change

Valley’s Anticoagulation Clinic’s name is changing! Beginning January 24, 2024, the name will change to Medication Management Clinics. The name change reflects expanded services to be offered at the clinic in the future. These expanded services will not be live on January 24 but will be announced at a later date. Current patients of the Anticoagulation Clinic will continue to receive the same service for their anticoagulation therapy.

Along with the clinic name change, the current referral to the Anticoagulation Clinic will be renamed to Medication Management Referral.  The new Medication Management referral will include “Anticoag” and “Anticoagulation” as synonyms when searching for referrals. A screenshot of the new referral is included below.  At go-live, we will not be accepting referrals for Medication Management. Information will be sent out once ready to accept such referrals.
