Health Coaches Available for Your Patients

Valley’s Volunteer Health Coach Program has been providing support for our primary care patients for five years!

The University of Washington now promotes our volunteer program to their pre-health, social work, and public health students! We need more patients for these volunteers to serve!

We serve your patients who live in King County, and:
+ Have one or more chronic disease for which a measurable improvement in clinical outcomes may be achieved through lifestyle change (typically, but not limited to: pre-diabetes and diabetes, obesity, hypertension, hypercholesteremia, COPD, CHF, sleep apnea, etc).
+ Have the ability and desire to set and work toward achieving health-related goals.
+ Can commit to weekly conversations with a volunteer health coach.

We provide whole-person care and work with our patients to address barriers to care caused by social drivers of health (SDoH).

Please know that your patients with active addictions or current unmet mental health therapy needs are best served through referrals to licensed staff. It is not within the scope of our volunteers to support these patients.

We recently worked with analysts from the University of Washington and found our participating patients experience statistically relevant improved clinical outcomes!

Please refer your eligible and interested patients through the Epic referral process or by sending a staff message directly to Laurie King RN, Health Coach Program Manager.
