A Safety Event Debrief is a summary of a root cause analysis and actions put into place after a safety event at VMC. Sharing these lessons learned from safety events and the plan for improving our processes at a system level, rather than focusing on individual failures is an important part of being a high reliability organization and of reaching our goal of Zero Harm.
