The Desimone Levee on the Green River failed and is under repair. The hospital is above the flood zone and currently in no danger of flooding. We evacuated our Time Square and Kent Station Clinics and our Lind Avenue locations. Several locations are experiencing staffing issues due to severe traffic impacts. Impacted patients are being contacted to reschedule appointments. Please be safe, do not drive or walk through standing water, and call 9-1-1 if you need emergency evacuation assistance.

Asthma Action Plans are developed in collaboration with the patient (and caregiver/family) and Physician/APP and include information and guidance on routine daily care and management of exacerbations. To best support the health and needs of our patients with asthma, existing Asthma Action Plans should be reviewed at least annually and with each acute exacerbation visit.
When utilizing this form for patients, page one can be filled out with the patient to provide their specific plan, and then page two is included to provide additional information and resources. This form also includes a barcode as the first page should be scanned in, which will allow us to track completion of the plan (page two does not need to be scanned).
This form currently lives in the Medical Records Forms library, and can also be accessed in Epic through the Clinic Provider Learning Dashboard. Thanks to Marketing for their help in developing this patient education tool.
