The VMC Referral Guide was recently expanded to add some new clinics and services, including:

• Children's Therapy
• Infusion Services
• Lifestyle Medicine & Outpatient Therapy
• Physical Medicine & Rehabilitation Services
• Wound Care Clinic

This resource is a directory specifically for clinicians and support staff who refer patients for VMC specialty services. Each clinic listed in the Guide includes:

• Building locations, addresses, main phone and back line numbers, and fax numbers
• Provider names, credentials, and specialties (Note: some advanced practice providers may not be listed if they don’t see new patients in clinic)
• Conditions treated and services offered
• Advance work-up requests, minimum age requirements, and any special instructions

As you’ll see on the Referral Guide webpage, each clinic listing can be opened as an individual PDF document, while the complete Guide is also available for those who would like a print-ready full document. The Referral Guide is intended for hospital and clinical support staff only; and is not intended for sharing directly with patients. The next version of the Guide will include ancillary services and primary care clinics.
