Every staff member and provider at Valley has a role in safety—your input helps us get better! The Culture of Safety Survey began in 2008, and has been conducted every two years since, providing valuable insights into opportunities for improvement and areas working well that we want to sustain.

The survey will be sent to all staff and providers via e-mail on Monday, Oct. 18. The survey will be live through Friday, Nov. 5. The only way to access the survey is through the e-mail you’ll receive from Survey Monkey.

Questions on this year’s survey will help us evaluate our work in areas including:

• Teamwork
• Staffing and work pace
• Response to error
• Communication about error
• Improving efficiency and waste reduction

This will be the first Culture of Safety Survey we’ve conducted since the pandemic began. Examining Valley’s Culture of Safety through the COVID lens will help us progress on our journey of high reliability.

This is an anonymous survey. There may be some questions that don’t make sense for your specific work area, and that’s ok! Just skip over the ones that don’t pertain to you. All feedback, from both clinical and non-clinical staff, is valuable.
