Advance care planning (ACP) is a process that supports adults at any age or stage of health in understanding and sharing their personal values, life goals, and preferences regarding future medical care. This recorded 3-part series is about ACP in the acute care setting, which focuses primarily on working with and finding surrogate decision makers, and participating in goals of care conversations with patients and families. While the inservice primarily reflects inpatient processes and case examples, this information can be valuable to caregivers in many different settings.
