Patient Health Coaching Program: All Insurance Types Now Considered

UPDATE: The cohort for July-December visits is now full and is accepting referrals for the January-June cohort.

Do you have patients who could benefit from the assistance of a health coach? Valley's volunteer Health Coach Program is currently accepting referrals of patients.

Patient eligibility:

  • One or more chronic disease for which a measurable outcome improvement may be achieved through lifestyle changes
  • Desire to make lifestyle changes and work toward health-related goals
  • Access to MyChart and the internet (for Zoom calls)
  • Medicaid or Medicare (priority, but other plans may be considered)
  • At risk as defined by:

--Valley risk score of 24 or higher of utilization (frequent ED visits, inpatient admissions)
--Lack of continuity (missed appointments, no follow up on referrals)
--Need for connection to community resources and referrals
--Difficulty with managing chronic disease

Referrals are easy! Simply send a staff message in Epic to Laurie King, RN and we will review the chart and reach out to your patient. (Stay tuned: You will soon be able to refer patients in Epic.)

The volunteer Health Coach Program is a FREE service that supports your patient as they work toward their self-management goals and serves as a liaison with, and extension of, the primary care team.
