NOTE: Medicaid insurance plans require a referral with health plan authorization for stage 2 including documentation of the patient having 5% weight loss, 12 nutrition visits in six months, and a psych evaluation (more details here). Representatives at the centers above said they will use Epic Care Everywhere to procure the required patient records, but behavioral health records cannot be accessed so your clinic may be contacted to fax the psych evaluation.
