Advance care planning (ACP) is important work that starts with each of us. By encouraging our patients to let their wishes be known, we are giving their caregivers and their loved ones peace of mind in knowing and helping ensure important care decisions are honored.

During this workshop, participants will learn all about the process of Advance Care Planning: how to start the conversation, how to fill out documents, where to get them notarized and how best to get to them quickly if needed.
