Why are there two plans?

  • Southlake Clinic physicians joined Valley Medical Center Clinic Network in the multispecialty expansion (MSE). These physicians have a separate incentive plan that is based the calendar year. Calendar year 2020 is the last full year of this plan.
  • The Legacy plan is the Clinic Network Plan and is fiscal calendar year based.
  • The provider incentive workgroup has been working to align the incentive measures of both plans.

Provider Engagement Metric:

Will providers be reminded to participate in the quarterly reflections?

How do I know if I should choose the reflection option?

  • You will receive monthly patient experience reports about your performance, based on your performance you can opt into the reflection activity. You need to submit a reflection in each quarter beginning quarter 1.
  • You can assure you get credit for this metric by doing the quarterly reflection.

Influenza Quality Metric:

What actions count for this measure?

  • Document counseling in the progress note by using .fluvaccinecounseling.
  • Update the Health Maintenance section with outside vaccines. This can also be done by the patient in MyChart.
  • Administer the vaccine.
  • Add flu allergy to the Allergy section of the chart if the patient has an allergy.
  • Only Providers can add declined to the flu HM topic (we encourage this should be done rarely).

All Quality Improvement Committees are focusing on flu vaccination and standard clinical workflows to support this work. Stay tuned!

When is the measurement period for the flu metric?

  • 9/1/20 (or when flu vaccines are available in clinics) thru 3/31/21

What patients are included in the denominator?

  • All unique patients age 6 months and older with an office visit or telehealth visit during the measurement period of 9/1/20-3/31/21.

Patient MyChart Utilization:

What counts for this measure?

  • If a patient reads a message from the clinic
  • If a patient sends a message to the clinic
  • If a patient reviews their lab results
  • If a patient has a telehealth visit

What is not included for this measure?

  • Telephone calls
  • Prescription refill requests

Which patients are included in this measure?

All unique patients seen in clinic for an office visit or telehealth visit during the fiscal year 7/1/20-6/30/21. Once a patient has been seen once, they are then included in your “pseudopanel.” All activity over the year of this patient is counted in the numerator.
