Clinic Network - Target: BP American Heart Association & American Medical Association Gold Level Recognition
It was a team effort for Valley Medical Center's Clinic Network to meet the Target: BP standard. The Ambulatory Care Pathway committee and multidisciplinary/multispecialty work group worked to standardize blood pressure targets and guide appropriate medication management.
Informatics developed the tools in Epic that hardwire and support the care pathways. Valley Medical Center's Professional Development Specialist and Training Coordinator promoted accurate and consistent blood pressure measurement through training. The Primary Care, Specialty Care, and Urgent Care Quality Committees embraced a second BP initiative in 2018 - 2019 to identify false positive elevated BPs. These combined efforts helped improve the accuracy of BPs recorded in the patient record and insured the use of evidenced based guidelines to improve our community's blood pressure, one patient at a time.

The Ambulatory Care Pathway committee adopted the American Heart Association & American College of Cardiology Blood Cholesterol Guideline. Informatics launched a risk estimator in Epic. Providers and care teams were educated on appropriate statin intensity for patients with or at risk of heart attacks and strokes. These professionals and clinic teams demonstrate dedication to helping our patients achieve the best possible outcomes.
