Valley Medical Center is excited to part of the UW Medicine Post-Acute Care (PAC) Network, offering enhanced clinical services and high-quality care for our patients who require post-acute care after hospitalization.

How is Valley Medical Center participating in this initiative?

  • Partnering with seven high-quality South King County skilled nursing facilities (listed below)
  • Placing a clinical team in select, high-volume, skilled nursing facilities (SNFs), with a physician and nurse practitioner team
  • Improved quality of transition to SNFs: Implementing a nurse-to-nurse warm handoff and improving documentation timeliness
  • Initiated readmission and post-discharge ED visit root cause analyses to identify areas of opportunity
  • Piloted technological initiatives including CarePort for patient family education and AIDA for adult family home searches
  • Identifying and collaborating with partners for home health, home care and adult day health
